Lyman is no longer accepting paper notes or emails for attendance. All notes must be submitted using Lyman's Attendance Google Form. Please use the QR code below or click here to access the Attendance Google Form.

Five (5) parent notes will be accepted per semester to excuse students’ absences that result from student illness. If a student exceeds five absences in a semester, the only notes that will be accepted to excuse student absences are as follows:

• Medical treatment provided by a licensed health professional via an official document. This includes, but is not limited to, a doctor, orthodontist, dentist, chiropractor, psychologist, vision, etc.
• Observance of religious practices or a religious holiday.
• Law enforcement order or court subpoena via a subpoena document. 
• Death of a family member via a death certificate, funeral program, obituary, etc.
• Traffic accident that directly involved the student via an accident report.
• College visits for Junior and Seniors only. 2 days per year via official document.

All excuses must be submitted via the Attendance Google Form within 5 school days of the student returning to school. After this date notes will NOT be accepted, and the absence will be recorded as unexcused. 

***All excuse notes must include the following: 

• Date 
• Student ID 
• Student Last Name and First Name (as it appears in Skyward—no nicknames) 
• Note if the absence is a full or partial day with the time denoted  

Seniors will not be able to participate in Senior Privileges if they exceed 10 unexcused absences and will forfeit their opportunity to attend Prom or other senior activities.
